De gewelddadige wormen hebben weer de beschikking over bizar wapentuig om hun vijanden een kopje kleiner te maken. Er zijn zes eilanden waarop kan worden gevochten. Deze levels zijn naar eigen smaak aan te passen en in te richten. Ook de wapens en oorlogsaccessoires zijn door de speler zelf samen te stellen. Worms Battle Islands kent dertig campagnemissies en dertig Puzzle Challanges voor n speler. De game is online tegen andere spelers te spelen. Er zijn vijf verschillende multiplayermodi waarin je de wormenoorlog kunt uitvechten.
